Student Information Release -I hereby give consent for the release of student information as it applies to school related activities such as the school directory, yearbook, athletics, musical programs, honors and awards, drama productions, graduation, leadership and school-related events.

This release shall not apply to confidential student transcript records including grades, attendance, test results, and health records

This consent will remain in effect until permission is revoked by the parents/guardians requesting in writing such revocation.

* Photograph Release -I hereby give my consent for Christ Lutheran School to reproduce my son's/daughter's image through photography and videography for use in school publications and for assignments to create multimedia projects.

This consent will remain in effect until permission is revoked by the parents/guardians requesting in writing such revocation.

Posting of a student's name, with an associated photograph will only be permitted where such identification is intrinsic to the posting. Additional consent via phone call or email will be confirmed prior to submission.
My child's image may become part of a public presentation including, but not limited to, displays such as the school internet website, Facebook, school brochures and programs, press releases, and newspaper articles

No - If a parental consent has not been given, no images or information on that student will be posted on our website or Facebook page. Nor will any image of the student will be allowed in classroom DVD's, or on bulletin boards outside of the classroom. Photo placement in the yearbook will be limited to the student headshot only—no candid pictures

Acceptance Policy

Christ Lutheran School admits students of any race, color and national or ethnic origin.

Because we want children to be successful at Christ Lutheran School, all new students will be accepted on a probationary stat us. There will be an ongoing review of CLS teachers and administration regarding the student's academic performance, behavior, effort, and attendance. Should there be questions about school/student fit, parents will be contacted for a conference. Christ Lutheran School reserves the right to rescind acceptance if concerns persist.

New students who apply for admission at Christ Lutheran School are notified of acceptance on an ongoing basis, pending completion of the application process. If your child has been accepted, but the final transcripts reveal any new information which would adversely affect your child's consideration for enrollment, we reserve the right to review our initial acceptance.

Prompt payment of tuition is required for all families.

I promise that if my child is admitted to Christ Lutheran School that I will sincerely and faithfully have the child apply maximum effort to the studies assigned and carefully observe the rules and regulations of the school.

I promise to be responsible for the payment of all items on my statement in a timely manner and within the school year in which the invoices have been incurred.

I certify that the information in this application is complete, true and accurate to the best of my knowledge. I realize that my failure to provide accurate information could jeopardize my student's enrollment at Christ Lutheran School
